- At 20wks-15% diastolic notch present
- At 24wks- 5% retain diastolic notch
- Placental side Uterine Artery-better predictor
- +/-Notch is the best predictor,
specially with high RI
- NOTCH IS BETTER PREDICTOR THAN RAISED RI
- Raised RI without NOTCH - did not have that
bad course of pregnancy in one study,
- Normal Ut Art flow waveform + low RI-very reassuring
- Abnormality >>impaired trophoblastic invasion
at risk for PE, IUGR, Abruptio placenta, PIH
- UNILATERAL NOTCH :
ALSO ABNORMAL, risk is somewhat lesser.
CONCLUSION :
FOCUS ON THE NOTCH
(UNILATERAL OR BILATERAL) AND LESS
ON RI AND PUT THAT PT AS HIGH RISK
IF NOTCH SEEN |
